29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. -Employees were roofing a residential structure while not using any type of fall protection system. Potential fall height was measured to 19' with roof pitches of 5/12 and 6/12.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: -Employees were using a JLG SkyTrak rough terrain forklift for access to upper levels while not using a suitable work platform, exposing themselves to fall hazards, struck-by hazards and crushing hazards.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). The fiberglass extension ladder was being used at a height to base ratio of approximately 1:1 exposing the employee to a potential fall height in excess of 8'.
