29 CFR 1910.134(m)(2)(i):     The employer did not establish a record of the qualitative and quantitative fit tests administered to an employee which included the information required by 29 CFR 1910.134(m)(2)(i)(A) through (m)(2)(i)(E):    On or about November 29, 2011 Guardian Polymers, inc. did not establish a record of fit testing for employees required to wear respirators exposing employees to the hazards of not having a properly fitting respirator.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c)
