29 CFR 1910.132(d)(1)(i): The employer did not select the personal protective equipment (PPE) that properly fits each affected employee:    (a) Thermo Fluids Inc., 6350 Sheridan Blvd., Arvada, CO 80003: On or before November 30, 2011, the employer did not select and have each affected employee wear reflective safety vests during oil collection. This condition created an struck-by hazard when an employee was run over by a vehicle while performing oil and water collection operations at a gas station on November 29, 2011.     Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
