29 CFR 1910.147(c)(1):     Energy control program. The employer shall establish a program consisting of energy controlprocedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine orequipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall beisolated from the energy source and rendered inoperative.    In the workplace, the employer had not established a lockout/tagout program for servicing and/or maintenance of machines and equipment such as, but not limited to press brakes, and saws.

One or more methods of machine guarding shall be provided to protect the operatorand other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips andsparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    1. In the workplace, the Johnson Horizontal Bandsaw had an approximately 24" x 10" top opening access to the blade which was not guarded.    2. In the workplace, the Johnson Horizontal Bandsaw had an approximately 7" portion of unused blade which was not guarded.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards,shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    1. In the workplace, the point of operation on the Chicago Press Brake model 131, serial number LW6964, was not guarded.    2. 29 CFR 1910.252(b)(2)(iii):     Protection from arc welding rays. Where the work permits, the welder should be enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide (an important factor for absorbing ultraviolet radiations) and lamp black, or shall be enclosed with noncombustible screens similarly painted. Booths and screens shall permit circulation of air at floor level. Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles.    In the workplace, the employer did not install protective screens or other equivalent protective devices to protect employees from welding flash.
