29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels were not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:    a)  Michael Shefferly on or about December 2, 2011, at the address of 2301 Nebraska Avenue, Toledo, OH:  The employer did not assure an employee was protected from fall hazards while assisting in the roofing process on a commercial building.  The employee was exposed to a 10 foot fall to a rubber roof below.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards.  Employees were not trained to recognize the hazards of falling and in the procedures to be followed in order to minimize these hazards:    a)  Michael Shefferly on or about December 2, 2011, at the address of 2301 Nebraska Avenue, Toledo, OH:  The employer did not assure an employee had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing various roofing operations.
