Abatement Note: The employer shall verify that the required workplace assessment has been performed through a written certification that shall include the following: 1) Identifies the workplace evaluated; 2) The person certifying that the evaluation has been performed; 3) The date(s) that the hazard assessment was performed; and 4) Which identifies the document as a certification of hazard assessment. 29 CFR 1910.133(a)(3): The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses. (a) Jordan Marble & Granite, LLC, 512 W. 67th St., Loveland, CO 80538: On and before December 7, 2011 an employee was wearing his personal prescription glasses without wearing eye protection over the prescription glasses. The employee was using a Makita Grinder and a grinding stone without proper machine guarding. This condition exposes the employees to hazardous conditions associated with flying chips should the grinding stone become fractured and break apart.Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet"). Note: In lieu of wearing protection over prescription glasses, the employer can provide the employee with prescription safety glasses.

Adjustment of the guard shall be such that pieces of an accidentally broken wheel will be deflected away from the operator. (a) Jordan Marble & Granite, LLC, 512 W. 67th St., Loveland, CO 80538: On and before December 7, 2011 an employee was using a handheld Makita grinder that was unguarded. The employees were using the grinder with an abrasive grinding stone. This condition exposes the employees to hazardous conditions associated with flying chips should the grinding stone become fractured and break apart.
