29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:    (a)  BLM Roofing, 221 Mesa Street, Brighton, CO 80601:  On and before December 7, 2011, the employer did not ensure that employees were protected from injury, in that seven employees were removing shingles from a 6-in-12 pitch on a two-story residential home were not utilizing a fall protection system. This condition exposed employees to a fall hazard above 6 feet.

This condition exposed employees to a eye hazard.

This condition exposed employees to a electrocution hazard.
