On or about December 7, 2011, at the above addressed jobsite, employees performing siding operations from a ladder jack scaffold, approximately twenty (20) feet four (4) inches above the ground level, were not provided with a ladder to safely access the work platform, thereby exposing the employees to falls.

On or about December 7, 2011, at the above addressed jobsite, employees performing siding operations from a ladder jack scaffold, approximately twenty (20) feet four (4) inches above the ground level, were not protected from falls by the use of personal fall arrest system(s).
