29 CFR 1926.1412(f)(1): At least every 12 months the equipment must be inspected by a qualified person in accordance with paragraph (d) of this section (each shift) except that the corrective action set forth in paragraphs (f)(4), (f)(5), and (f)(6) of this section must apply in place of the corrective action required by paragraphs (d)(2) and (d)(3) of this section.    (a)	Hudspeth & Associates, Inc. at Fruita Refinery 1493 Hwy 6 & 50, Fruita, CO 81521: On and before 11/9/11, the employer did not protect employees from hazards created by operating an American truck mounted crane model #7520 without conducting a comprehensive annual inspection by a qualified person. This condition exposed employees to a crush by hazards.

(a)	Hudspeth & Associates, Inc. at Fruita Refinery 1493 Hwy 6 & 50, Fruita, CO 81521: On and before 11/9/11, the employer did not protect employees from hazards created by operating an American truck mounted crane model #7520 while operating the crane in a 4 part line configuration which created an insufficient amount of line in the drum. This condition exposed employees to a crush by hazards.

29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe while rigging up the chokers to pick materials from the derrick platform.      (a)	Hudspeth & Associates, Inc. at Fruita Refinery 1493 Hwy 6 & 50, Fruita, CO 81521: On and before 11/9/11, the employer did not protect employees from hazards created by operating an American truck mounted crane model #7520 while rigging up the crane to lift materials with chokers that were worn and missing tags to determine the safe working loads. This condition exposed employees to a crush by hazards.
