29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:     (a) On the north side: On or about 12/6/2011, and at times prior thereto, employees working on the second level deck of the tubular welded frame scaffold  that was not completely decked, it had gaps of 4 inches on each side, were exposed to a fall hazard.     Note: Abatement certification and documentation are required.      The employer, Shane Demler Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1)which was contained in OSHA inspection Number 311582779 Citation number 1, item number 2a and was affirmed as a final order on 06/12/2009, with respect to a workplace located at 900 Environmental Way, Idaho Falls, ID 83401.

29 CFR 1926.451(e)(1): Scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. Crossbraces were used as a means of access:    a) On the north side: on or about 12/6/2011, and at times prior thereto, employees were not provided with a safe means of accessing the second and third level decks of the tubular welded frame scaffold.    Note: Abatement certification and documentation ARE required.    The employer, Shane Demler Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1)which was contained in OSHA inspection Number 311582779 Citation number 1, item number 3 and was affirmed as a final order on 06/12/2009, with respect to a workplace located at 900 Environmental Way, Idaho Falls, ID 83401.

29 CFR 1926.451(g)(1): Employee's on a scaffold platform more than 10 feet (3.1 m) above a lower level were not protected from falling to a lower level.  Fall protection for scaffolding must meet requirements specified in 29 CFR 1926.451(g)(1)(i) through (vii):     a) On the north side scaffolds: On or about 12/6/2011, and at times prior thereto, employees working on the tubular welded frame scaffolding at a height of approximately 11.5 feet were not protected by guard rails and end rails.     Note: Abatement certification and documentation are required.    The employer, Shane Demler Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1) which was contained in OSHA inspection Number 311582779 Citation number 1, item number 2b and was affirmed as a final order on 06/12/2009, with respect to a workplace located at 900 Environmental Way, Idaho Falls, ID 83401.
