29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards,shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. At the MeGlennon Metal Products facility located in Columbus, Ohio the employer had two press brakes that were operated on a daily basis, on two different shifts that were not guarded. The press brakes were used to bend the edges of spun metal products which had employees handling material within 2-3 inches of the point of operation.

29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. a. At the McGlennon Metal Products facility the employer had an injury that occurred on or about March 11, 2010 on their 2010 OSHA 300 log that resulted in days away from work and restricted work activity. The incident was not adequately described in column (f) in that the employer did not describe the injury or the object/substance that directly injured the employee.
