Pickens Kane Moving and Storage failed to complete and send to OSHA, or its agent, the "OSHA Occupational Injury and Illness Data Collection Form, 2010" within 30 days of the date Pickens Kane Moving and Storage received the Form, which was mailed to Pickens Kane Moving and Storage on or around July 6, 2011. Abatement documentation is required for this item in accordance with requirements of 29 CFR 1903.19(d).
