29 CFR 1910.212(a)(1)  One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points:    a)  On or about January 10, 2012, the guide wheel above the puller, approximately 64 inches above floor level, used to lead the wire running to the puller, was open exposing workers to an ingoing nip point.    b)  On or about January 10, 2012, the guard over the puller machine was a flip up guard, observed hanging in place over the puller operation, and not secured in place as required.

29 CFR 1910.219(c)(2)(i)  All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, were not protected by a stationary casing enclosing shafting completely:    On or about January 10, 2012,  the drive shaft for the extruder motor, at floor level, next to the cat-a-puller line, was open and not protected by an enclosure as required.

29 CFR 1910.219(f)(1)(i)  Gears were not guarded by a complete enclosure:    a)  On or about January 10, 2012,  the entrance to the cat-a-puller was open, measuring approximately one foot by one foot square, exposing the operators to the hazard of being pulled into the machine.    b)  On or about January 10, 2012,  at the end of the extrusion line the powered gears for the take up operation, near floor level and at each side of the take up wheels, were exposed with openings of approximately 24 inches by 8 inches.

29 CFR 1910.305(g)(2)(iii)   Flexible cords and cables were not connected to devices and fittings so that the strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws:    On or about January 10, 2012,  the  flexible power cord to the extruder was not connected by a strain relief as required, observed hanging from the entry to the extruder.
