29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan shall be reviewed and updated at least annually and whenever necessary to reflect new or modified tasks and procedures which affect occupational exposure and to reflect new or revised employee positions with occupational exposure: At the work site located at 41 Commerce Park Drive, Worthington, Ohio, the employer did not annually review the Exposure Control Plan. There was no evidence of review or updates for over three years.

29 CFR 1910.1030(g)(2)(ii)(B): The employer shall train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program. Training shall be provided at the time of initial assignment to tasks where occupational exposure may take place, and at leas annually thereafter. At the work site located at 41 Commerce Park Drive, Worthington, Ohio, the employer did not ensure employees with occupational exposure to blood borne pathogens received annual training as required by the standard.
