29 CFR 1910.157(e)(2):     Portable fire extinguishers were not visually inspected at least monthly:    Carrolls Truck Parts, located at 2064 Nazor Rd., Galion, OH: On or about January 17, 2012, the employer did not ensure that portable fire extinguishers on site were inspected monthly in order to make sure that extinguishers were fully charged and in operable condition, exposing employees to hazards associated with fire suppression and operation of fire suppression equipment.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The employer did not ensure that each powered industrial vehicle operator received formal instructions and practical training on how to operate a powered industrial vehicle, as well as the operators performance was not evaluated. 29 CFR 1910.178(p)(1):     Where powered industrial trucks were found to be in need of repair, defective, or in any way unsafe, the trucks were not taken out of service until they had been restored to safe operating condition:    A.	Carrolls Truck Parts, located at 2064 Nazor Rd., Galion, OH: On or about January 17, 2012, the employer failed to remove the powered industrial vehicle Volvo BM (Type L90, Cab Serial # 16568) from use based on mechanical defects such as, but not limited to, broken rear lights. Employees were exposed to struck-by and caught in between hazards.    B.	Carrolls Truck Parts, located at 2064 Nazor Rd., Galion, OH: On or about January 17, 2012, the employer failed to remove the powered industrial vehicle FiatAllis (Model R15B/2T, Serial # 595535) from use based on mechanical defects such as, but not limited to, transmission oil leak and broken rear lights. Employees were exposed to struck-by and caught in between hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Employees were exposed to a risk of serious injuries if the cylinders fell over, the valve assemblies were knocked off, and the escaping gas ignited, or if the escaping gas propelled the cylinders into the employees.
