29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    Worker(s) were exposed to a fall hazard of 6'8" while accessing a temporary work-platform inside the Incoe injection molding press. Employee(s) had to step over an 18 inch gap from the traverse cylinder to access the work-platform.     Abatement Certification and Abatement Documentation Required.

29 CFR 1910.28(a)(11): All planking or platforms were not overlapped (minimum 12 inches) or secured from movement:     On or about January 19th, 2012 an employee fell six (6) feet eight (8) inches off a temporary platform inside injection molding press 10-1 (T01), injuring his shoulder/chest region and the back of the head. The platform was not secured properly and it slid across the tie-bars as the employee attempted to step onto it.     Abatement Certification and Abatement Documentation Required.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    On or about January 16th, 2012 an employee made contact with a live electrical transformer inside the enclosure of the #2208 Cutter that was not guarded against accidental contact. The employee was hospitalized and suffered third degree burns to his right hand and forearm.     Abatement Certiification and Abatement Documentation Required.
