29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:    1) On January 31, 2011, employees in the grinding department were exposed to noise at levels that exceeded an 8 hour TWA of 85 decibels.  The employer had not established a hearing test program.    a) An employee working in the grinding department was exposed to approximately 89.7 dBA of noise during a 450 minute sampling period.  The calculation assumed zero exposure for the unsampled portion of the shift.    b) An employee working in the grinding department was exposed to approximately 91.3 dBA of noise during a 440 minute sampling period.  The calculation assumed zero exposure for the unsampled portion of the shift.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    1) On January 31, 2011, and at prior times, employees were exposed to amputation hazards while operating bandsaws in the grinding department.  The guards on the bandsaws were not adjusted to cover the unused portion of the saw when aluminum castings were cut.
