29 CFR 1926.451(d)(1):  All suspension scaffold support devices, such as outrigger beams, cornice hooks, parapet clamps, and similar devices shall rest on surfaces capable of supporting at least 4 times the load imposed on them by the scaffold operating at the rated load of the hoist (or at least 1.5 times the load imposed on them by the scaffold at the stall capacity of the hoist, whichever is greater):    Parapet clamps supporting a two-point suspension scaffold rested on an unreinforced masonry parapet wall.

29 CFR 1926.451(d)(3)(i):  Before the scaffold is used, direct connections shall be evaluated by a competent person who shall confirm, based on the evaluation, that the supporting surfaces are capable of supporting the loads to be imposed:    Before the two-point suspension scaffold was used on 01/24/2012, no one looked at the scaffold, its direct connections,  or the parapet wall for its supporting capacity.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:    Employees were not trained on the correct use and operations for using SAFEWAZE rope grabs.  Instructions imprinted on the metal rope grab stated "to be used with 3 foot lanyard".  The lanyards used by the employees measured 5 feet 3 inches.

29 CFR 1926.103 as set forth in 29 CFR 1910.134(c)(1):  In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use:    There was no written Respiratory Protection program for masons using 1/2 mask, negative-pressure respirators during operations that include working with products containing silica and/or portland cement.
