29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    Employees were exposed to fall hazards which was at least 12 feet to the ground while performing roofing work on an 8 in 12 pitch roof and were not wearing a safety harness with attached lanyard to the anchor rope as part of a personal fall arrest system, or by using other means of fall protection.

29 CFR 1926.1060(a):     The employer did not provide a training program for each employee using ladders and stairways, that enabled each employee to recognize hazards related to ladders and stairways, and that trained each employee in the procedures to be followed to minimize those hazards:    The employer did not have a training program that would  enable each employee to recognize the hazards of working hazardously on ladders was insufficient to train each employee in procedures for the use and placement of ladders, to be followed in order to minimize fall hazards.  An employee was exposed to fall of up to 30 feet high while using an extension fiberglass ladder.  The ladder was on the 8 in 12 pitch, secured with a board nailed to the roof to keep it in place and with a wire tie in the mid-section to secure the ladder.  Another employee was holding the ladder at the midsection for additional stabilization.
