29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provisionin paragraph (b) of this section provides for an alternative fall protection measure.     A. JRS Construction, Ltd. worksite located at 6853 Deer Ridge, Maumee, Ohio: On or about January 31, 2012, the employer did not ensure employees wore fall protection while installing plywood sheathing on the single story multi-dwelling residential structure. Employees were exposed to a fall distance of approximately 8 feet 6 inches to the lowest level from the 5/12 pitch roof.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    A. JRS Construction, Ltd. worksite located at 6853 Deer Ridge, Maumee, Ohio: On or about January 31, 2012, the employer did not ensure employees were wearing proper eye protection while operating a pneumatic fastening device to secure sheets of plywood on a single story multi-dwelling residential structure. The employee was exposed to potential flying object hazards while operating the pneumatic fastener.    JRS Construction, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.95, which was contained in OSHA inspection number 314673369, citation number 1, item number 1 and was affirmed as a final order on July 30, 2010, with respect to a workplace located at Brunswick, OH.
