29 CFR 1926.100(a):  Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    Location: 152 W. 36th St., New York, NY.    a) Employer failed to ensure that employees working at the gut/rehab construction project wore protective helmets.  The site had over head hazards, such as, but not limited to, construction materials and tools; on or about 1/24/12.
