29 CFR 1910.119(d)(3)(i)(B):  Information pertaining to the equipment in the process did not include accurate piping and instrument diagrams (P&ID's):    a) On or about January 31, 2012 and times prior the employer's piping and instrumentation diagrams were not accurate in that equipment was found during the walkthrough that was not identified on the P&ID. A 1 ball valve was found on a 2 line entering US-102 that was not listed on the P&ID.     b) On or about January 31, 2012 and times prior the employer's piping and instrumentation diagrams were not accurate in that equipment was found during the walkthrough that was not identified on the P&ID. A pressure relief valve (PRV#2) was identified on the VD-103 south blow case that was not listed on the P&ID.    c) On or about January 31, 2012 and times prior the employer's piping and instrumentation diagrams were not accurate in that a pressure relief valve (PRV) was listed on the P&ID and were not identified during the walkthrough. The PRV associated with the compressor inlet gas process was listed on the P&ID but could not be found during the walkthrough.    d) On or about January 31, 2012 and times prior the employer's piping and instrumentation diagrams were not accurate in that equipment numbers found during the walkthrough did not correspond to the equipment numbers listed on the P&ID. The tag numbers found on the compressors pressure relief valves did not match the numbers listed on the P&ID.    William Four Corners Llc dba Williams Field Services Koskie was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection Number 310593702, Citation Number 1, Item Number 1, and issued on 05/23/2008, with respect to a workplace located at 3746 County Road 307, Durango, CO 81301. This citation became final order on November 14, 2008.

29 CFR 1910.23(d)(1)(iii): On stairways less than 44 inches wide having both sides open, one stair railing on each side was not installed:    a) On or about January 31, 2012 and times prior employees were exposed to injuries from falling to the lower surface in that the stairway leading to the service platform VD-103 were not guarded on both sides.
