OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:     a) Mid Continent Energy, Inc, Rig #1, 70 Ranch, Kersey, CO: On and before February 9, 2012 the employer had not ensured that prior to pumping high pressure mud fluid through a pipe into the wellbore during drilling operations, that each section of line was adequately secured to prevent safe movement. This condition exposed employees to the hazard of being struck by the hose assembly in the event of a system failure.

This condition exposed employees to fire and explosion hazards.

29 CFR 1910.24(f): The employer did not furnish stair treads where the rise height and tread width were uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs:    a) Mid Continent Energy, Inc, Rig #1, 70 Ranch, Kersey, CO:  On and before February 9, 2012 the employer did not furnish stair treads where the rise height and tread width were uniform in that approximately four steps leading from the ground of the pad to the rig floor were dented and non-uniform. This condition exposed employees to a fall hazard.

29 CFR 1910.305(g)(2)(ii): The employer did not use flexible cords in continuous lengths without splice or tap:    a) Mid Continent Energy, Inc, Rig #1, 70 Ranch, Kersey, CO:  On and before February 9, 2012 the employer did not remove from service an extension cord used to power lights on a pump, in that the flexible cord had been spliced from a yellow and black cord. This condition exposed employees to electrocution hazard.
