OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: a) Mid Continent Energy, Inc, Rig #1, 70 Ranch, Kersey, CO: On and before February 9, 2012 the employer had not ensured that prior to pumping high pressure mud fluid through a pipe into the wellbore during drilling operations, that each section of line was adequately secured to prevent safe movement. This condition exposed employees to the hazard of being struck by the hose assembly in the event of a system failure.

This condition exposed employees to fire and explosion hazards.

This condition exposed employees to electrocution hazard.
