29 CFR 1926.501(b)(4)(iii): Each employee on a walking/working surface was not protected from objects falling through holes (including skylights) by covers:    a. Site- Employees were not protected from struck by hazards where tools and building materials including, but not limited too, buckets of fastening screws and washers were allowed to accumulate on top of metal grate covers over the sky lights which had holes 6 inches by 6 inches.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a. Roof- Employees accessing the roof were exposed to a fall hazard where the ladder did not extend 3 feet above the landing surface.
