29 CFR 1904.29(a):     The employer did not use an OSHA 300, 301 and 300A Form or equivalent.    a.) Mathews Dodge Chrysler Jeep, Inc. located at 1866 Marion Waldo Rd. Marion, OH 43302; On or about February 16, 2012, the employer did not meet the basic requirements for recording injuries and illnesses.  The employer failed to record all work related injuries for the calendar year 2010 on the OSHA 300 Logs in their entirety.
