29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 2/17/12, machine guarding was not provided for the polishing wheels of 3 unnamed polishers. Operators were exposed to flying chips and sparks during the polishing process, in additon to entanglement hazards due to the rotating arbor/spindle shafts, couplers, spindle end nuts, and the unused arbor/spindle.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about 2/17/12, a flexible cord, attached to the "rectifier", located in the Plating Room on the west wall, had pulled away from the strain relief device at the equipment plug, exposing employees to electrical shock hazards upon contact.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: On or about 2/17/12, a hazard assessment, designed to identify hazards at the workplace and used for selecting appropriate personal protective equipment (PPE) such as, appropriate eye protection, was not certified in writing that it was completed by the employer. NOTE: The assessment shall be written, titled, dated, and certified by signature. The required documented training shall include the following: 1) When PPE is necessary; 2) What PPE is necessary; 3) Limitations of the PPE; 4) How to properly wear and use PPE; and, 5) Inspection and care of the PPE.
