29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D when the employer determined that any voluntary respirator use was permissible:    (a)	Rock Solid Marble & Granite, LLC, 2001 E. 11th St., Loveland, CO 80537: On and before February 22, 2012, an employee was wearing a Moldex N95 respirator on a voluntary basis. The employer did not provide employees with the information contained in Appendix D.     Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.1200(h)(1):  Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety datasheets.    (a)	Rock Solid Marble & Granite, LLC, 2001 E 11th St., Loveland, CO 80537  On and before February 22, 2012, the employer did not provide employees with hazardous chemical training. The employer did not provide employees with training regarding the hazards of silica.
