29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Jobsite located at 4657 G Street, Omaha, NE and throughout the company:  Procedures were not developed to protect employees from unexpected energization, start up, or release of stored energy.  As of the date of the inspection, a specific procedure had not been developed for Proof Box #2, a piece of equipment that was involved in an accident where an employee energized the machine causing a second employee to become caught in a belt and pulley.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

This training should have included the energy sources in the workplace as well as the methods necessary for energy isolation and control.
