29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a) In the Production Area, a 33.5 inch by 50.5 inch section of the robotic fence guard was missing on Press 210.    (b) In the Assembly Area, the Hands Free Mechanism Press operating control did not prevent the operator's left hand from being between the press ram and the hands free mechanism. The gap between the press ram and the hands free mechanism is larger than 3 inches.           ABATEMENT DOCUMENTATION AND ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1910.147(c)(6)(ii): The periodic inspection certification of energy control procedures did not contain the required documentation on identification of the machine or equipment, inspection date, employees included, and the person performing the inspection:    (a) The Deluxe Plastics Lockout Tagout Authorized Personnel Sign-In sheets dates 08/31/11 and 09/01/11 do not include the identification of the equipment or machine included in the periodic inspection.         ABATEMENT DOCUMENTATION REQUIRED.
