29 CFR 1926.503 (a)(1) The employer did not provide a training program for each employee who might be exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.    a)  Voorhees construction site - Employees did not know how to properly protect themselves when exposed to fall hazards on the site that were up to approximately 36' in height, on or about 02/08/12 & 02/09/12.        NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a)  Southwest stair tower area - Five to six improperly protected employees were exposed to an approximate 24' fall to concrete, on or about 02/08/12;    b)  Southwest stair tower area - Employee without proper fall protection was exposed to an approximate 36' fall to the ground, on or about 02/08/12;    c)  East area of roof - Employee without proper fall protection was walking around on the plywood forms resting on the roll bars and was exposed to an approximate 36' fall to the outside of the building and 12' to the interior, on or about 02/09/12.    PREMIER HSFC CRANE & RIGGING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD, 29 CFR 1926.501(b)(4)(i) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313985426 CITATION NUMBER 01, ITEM NUMBER 003, ISSUED ON 06/14/2011.  NOTE:  FINAL ORDER DATE - 06/28/2011.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
