29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    a)  Maintenance Shop - On or about February 21, 2012, the Rockwell abrasive wheel grinder (Model # 438-021314-0180) was used with a tongue guard that was not adjusted to the required 1/4" distance exposing employees to struck-by hazards.    b)  Maintenance Shop - On or about February 21, 2012, the Baldor abrasive wheel grinder (Serial # G6-0443-0017) was used without tongue guards on the right and left abrasive wheels exposing employees to struck-by hazards.
