29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: location: exterior of house on or about: 2/27/12 a) Employees were using Werner portable metal extension ladders to perform roofing activities and to access the ladder jack scaffold. The lanyards, connecting the 2 flys of each ladder, were missing. NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, that enabled each employee to recognize hazards related to ladders and stairways, and that trained each employee in the procedures to be followed to minimize those hazards: location: exterior of house on or about: 2/27/12 a) An employee was using Werner portable metal extension ladders while engaged in residential roofing. The employee was not provided with ladder safety training. NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
