29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Ross Printing, Inc. at 550 E. 76th Ave., Denver, CO 80229:  On or about March 7, 2012, the employer did not ensure that employees were protected from the following machine guarding hazards:    1)  employees were exposed to ingoing nip point hazards for the coating cylinders on the Dorn SPE UV SeriesRoller Coater, model ARC-30, sn FOR 9065;    2)  employees were exposed to rotating parts hazards for the conveyor belt cylinder on the Dorn SPE UV SeriesRoller Coater, model ARC-30, sn FOR 9065;    3)  employees were exposed to rotating parts hazards for the drive wheels on the first Stahl folder;    4)  employees were exposed to ingoing nip point hazards for the drive cylinder on the first Stahl folder;    5)  employees were exposed to rotating parts hazards for the drive wheels on the second Stahl folder; and    6)  employees were exposed to ingoing nip point hazards for the drive cylinder on the second Stahl folder.    Condition exposed employees to amputation hazards.

29 CFR 1910.242(b):  Compressed air was used for cleaning purposes at more than 30 p.s.i. (when dead-ended):    a)  Ross Printing, Inc. at 550 E. 76th Ave., Denver, CO 80229:  On or about March 7, 2012, the employer permitted employees to use compressed air for cleaning purposes at 116 psi, when dead-ended, to clean the presses and the bindery machines.  Condition exposed employees to health hazards including embolisms.

29 CFR 1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedures and the requirements of this standard were being followed:    a)  Ross Printing, Inc. at 550 E. 76th Ave., Denver, CO 80229:  On or about March 7, 2012, the employer did not conduct periodic inspections of the energy control procedures.  There was no verification that lockout tagout procedures were in place for the maintenance, tool changes, and setup of the presses and bindery equipment.  Condition exposed employees to amputation hazards.    Abatement note:  29 CFR 1910.147(c)(6)(ii) also requires a certification to ensure that periodic inspections have been accomplished.
