(a) On or about 02/26/2012 at 615 Route 18 South, East Brunswick, NJ 08816: Employees working on lower levels of scaffold and at ground level were not provided with head protection while working directly below masonry tools, mortar pans and bricks.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports as follows:    (a) On or about 02/26/2012 at 615 Route 18 South, East Brunswick, NJ 08816: Employee worked from Tubular Welded Scaffold platforms which were not fully planked and were exposed to fall hazards of approximately 13' to the concrete ground below.

29 CFR 1926.451(e)(1): For scaffold platforms more than two feet above or below a point of access, portable ladders, hook on ladders, attachable ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personal hoist, or similar surface were not used.    (a) On or about 02/26/2012 at 615 Route 18 South, East Brunswick, NJ 08816: Employees were not provided with a safe means of access to the work platform located at approximately 13' from the ground below. Employees climbed the  Tubular Welded Scaffold Framework to access the working platform as a ladder or other safe means of access was not provided.

29 CFR 1926.451(g)(1): Each employee on scaffolds more than 10 feet were not protected from falling to the lower level:    (a) On or about 02/26/2012 at 615 Route 18 South, East Brunswick, NJ 08816: Employees working on Tubular Welded Scaffold System at heights greater than 13' were not provided with a fall protection system. No guardrails or other type of fall protection was provided on the working levels of the Tubular Welded Scaffold System.
