29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected fromfalling to that lower level. a) At the jobsite, on the 5th rise of tublear welded frame scaffolding structure, on or about 3/2/12: An employee was stratteling; from the outside, two scaffold ends at approximately 36 feet from the lower level, and without fall protection of any kind.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of anyanchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached. a) At the jobsite, on the 5th level of a tubulear welded frame scaffold structure, on or about 3/2/12: An employee was receiving material from a lift, with the top and midrail removed from the scaffolding, and wearing a personal fall arrest harness with lanyard, attached to the scaffolding, that was not designed to support at least 5,000 pounds of force.
