29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.        Employees were not given safe access to a fabricated frame walk through scaffolding.

29 CFR 1926.451(g)(4)(ii): The top edge height of toprails on all supported scaffolds were not between 38 inches (0.9 m) and 45 inches (1.2 m) above the platform surface:    Note:  For supported scaffolds manufactured and placed in service before January 1, 2000, and on all supported scaffolds the top edge height should be between 38 inches (0.9 m) and 45 inches.    Guardrails used on fabricated frame walk through scaffolding with a top rail height not between 38 inches and 45 inches.
