29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:        Employees at the job site located at 4340 McRee Ave, St. Louis MO on 7 March 2012 were not protected from falling object by protective hats.          Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.150(a)(4):     Firefighting equipment was not periodically inspected:  (a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY)          Employees at the job site located at 4340 McRee Ave, St. Louis MO on 7 March 2012 did not have a fire extinguisher that had been inspected in the last 12 months.  As a result the fire extinguisher had been discharged during past kettle operations and needed to be recharged.          Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.        At the job site located at 4340 McRee Ave, St. Louis MO on 7 March the employer did not ensure employees used or had available for use, required fall protection systems.         A citation was previously issued for a violation of 1926.501(b)(13) with a final order date of 27 August, 2010 located at 2220 Oregon Street, St. Louis MO for employees not using required personal fall protection equipment.  A recent inspection of the same establishment revealed a violation of the same standard for employees not using required personal fall protection equipment.  Since the conditions are substantially similar a repeat violation is being issued for the violative conditions.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
