29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation: a. Entire Facility - On or about March 15, 2012, the employer's OSHA 300 Log forms were missing information on column "F". The number of entries per year that were missing information were the following; 1. Two entries in the OSHA 300 Log form for the year 2010 were missing information such as the description of the injury or illnesses. 2. Five entries in the OSHA 300 Log form for the year 2010 were missing information such as the description of the object/substance that directly injured or made person ill. 3. One entry in the OSHA 300 Log form for the year 2011 was missing information such as the description of the injury or illnesses. 4. Four entries in the OSHA 300 Log form for the year 2011 were missing information such as the description of the object/substance that directly injured or made person ill. 5. Two entries in the OSHA 300 Log form for the year 2011 were missing information such as the parts of the body affected.

29 CFR 1910.1026(i)(4): The employer shall ensure that eating and drinking areas and surfaces are maintained as free as practicable of Chromium (VI): a. Employee's Lunch Room - On or about March 15, 2012, employees were permitted to consume food and/or beverages in an area that contained toxic material. The locations where toxic material was found are the following; 1. The door opener for the microwave (GE - Inver Tech) on the kitchen counter contained 0.0616 micrograms of chromium (VI). 2. The North West lunch table contained 0.0942 micrograms of chromium (VI). 3. The counter area next to the sink contained 0.1744 micrograms of chromium (VI).
