Rockwell, Oklahoma City, Oklahoma worksite: Pinion Design and Contracting employees were using a SkyTrak telehandler rough terrain forklift model 10054 not in accordance with manufacturer's design and operating instructions. a) SkyTrak outriggers were not extended out onto solid surfaces exposing employees to the hazards of a forklift tipping over. b) Material was being hoisted with the Skyrak without the proper hoisting attachment exposing employees to the hazards of of being struck by an unstable load.
