29 CFR 1926.451(g)(1)(i):     Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) On or about 03/20/12, at a residential home under renovation at 34 Richmond Ave., Buffalo, NY:  Two ladder jack scaffolds were erected on the south side of the house. One employee was on the "pik" of each scaffold and was painting the side of the house. Each employee was exposed to a fall of approx. 17 ft. to the ground. No personal fall arrest system or other fall protection system was utilized.    b) On or about 03/20/12, at a residential home under renovation at 34 Richmond Ave., Buffalo, NY:  One ladder jack scaffold was erected on the backside of the house. One employee was on the "pik" of that scaffold and was painting the back of the house. The employee was exposed to a fall of approx. 19 ft. to the ground. No personal fall arrest system or other fall protection system was utilized.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.454(a):     The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a) On or about 03/20/12, at a residential home under renovation at 34 Richmond Ave., Buffalo, NY:  Employees doing painting from the work platforms of three ladder jack scaffolds erected on this site were not trained about the nature of fall hazards associated with working from this type of scaffold, the pertinent OSHA scaffolding and ladder access  regulations, and the procedures to control or minimize those hazards.    ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1051(a):     Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:    a) On or about 03/20/12, at a residential home under renovation at 34 Richmond Ave., Buffalo, NY:  Two ladder jack scaffolds were erected on the south side of the house. Each scaffold had two support ladders. The scaffold platforms ("piks") were approx. 16 ft. above the ground and rested on metal brackets approx. 24 inches wide that were installed on  the "climbing side" of the support ladders. The employees accessed the scaffold platform by climbing the support ladders up to the metal bracket and then hoisting themselves up onto the scaffold platform. No seperate access ladder was provided.    b) On or about 03/20/12, at a residential home under renovation at 34 Richmond Ave., Buffalo, NY:  One ladder jack scaffold was erected on the back side of the house. This scaffold had two support ladders.
