29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about 3/20/12 at a rehabilitation project located at 908 Center Street, Lewiston, New York; employees working on the roof of a building with pitches of 12/12 and 5/12 were not utilizing fall protection.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) On or about 3/20/12 at a rehabilitation project located at 908 Center Street, Lewiston, New York; employees were using a 20 foot extension ladder to access the work area and carry materials up to the roof. The side rails of the ladder only extended 2 feet above the landing surface.    NO ABATEMENT CERTIFICATION REQUIRED
