29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Located at 217 Grand Street, Brooklyn, NY    On or about 3/22/2012    a)  Employees used an extension ladder to access 1st level mezzanine and the ladder did not extended at least 3 feet above the landing surface being accessed.

A Frame ladder as a lean against ladder to access 2nd floor landing.
