29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) On or about April 16, 2012, The employer did not ensure the scaffold, approximately twleve feet in height, was fully planked for employee safety while adding stucco to the rear of a buliding, exposing the employees to fall hazards while working. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the scaffold is fully and securely planked on all working levels for employee safety.

29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement: On or about April 16, 2012, the employer did not ensure the scaffold the employees were working approximately twelve (12) feet in height had stable footing to prevent a fall hazard to the employees while they applied stucco to the rear of the building. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the scaffold are properly erected with correct and stable footing.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. On or about April 16, 2012, the employer did not provide a means of egress for employees working on a scaffold over two tiers in height; the employees used the crossbars and plank rests for means of egress. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a means of egress is provided for employees working on a scaffold.
