29 CFR 1926.501(b)(13): Where the employer was engaged in residential construction activities 6 feet (1.8m) or more above lower levels employees were not protected from falling by guardrail systems, safety net systems, or personal fall arrest system:    a) On or about March 22, 2012, and at times prior, Thomas Construction, Inc., as controlling employer, did not ensure that Red Line Construction, Inc., a subcontractor, protected their employees from falls in excess of 6 feet while rolling floor joists.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:      a) On or about March 22, 2012, and at times prior, Thomas Construction LLC, as controlling employer, did not ensure that Red Line Construction, Inc., a subcontractor, protected their employees from falls in excess of 10 feet. Employees were using the braces to access the first floor in order to roll floor joists.
