29 CFR 1926.858(d):  During demolition operations the employer did not ensure that any structural member being dismembered shall not be overstressed:    a)  During demolition of the two story building on 3-21 a large crack developed in one of the three main steel beams (B girder) which support the 2fl.  The crack was at a weak point in the girder known as a splice connection which became overstressed when part of the girder was cut off.  Though the splice connection itself was not visible due to concrete encasing and was not shown on the demolition plan, the large crack was in plain view and the girder beam was visibly sagging in danger of collapse.  Employees were exposed to structural collapse hazard.      Location: 606 W 131 Street, New York, N.Y     On or about 3/22/2012       NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.859(g):  The employer did not ensure that:  During demolition, continuing inspections by a competent person shall be made as the work progresses to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material.  No employee shall be permitted to work where such hazards exist until they are corrected by shoring, bracing, or other effective means:    a)  During demolition of the 2 story garage building:  The employer did not inspect and discover a large crack in the B girder which had developed on 3-21.  On 3-22 the employer cut the next beam (C girder) which had a similar spliced weak point, resulting that the 2nd floor collapsed.      Location: 606 W 131 Street, New York, N.Y     On or about 3/22/2012       NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
