29 CFR 1910.243(c)(1):     General requirements. Abrasive wheels shall be used only on machine provided with safety guards as defined in paragraph (c) (1) through (4) of this section.    Jobsite located at 13010 F Plaza, Omaha, NE: The employer failed to ensure that portable grinding equipment was used with guards installed while being utilized.  On February 24,2012, an employee of Ribbon Weld was injured when a cut-off wheel, improperly installed on a portable grinder, failed and there was not a guard in place to protect the employee.  This grinder was utilized by employees of Omaha Track Equipment, the owners of the portable grinder.      Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.
