29 CFR 1926.403(i)(2)(i):     Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph.      a) Electrical distribution room - On or about March 20, 2012, employees were working in the area of energized 240 VAC electrical panels without a factory dead front and door assembly installed.    b) Facility wide - On or about March 20, 2012, employees were working in and around energized electrical receptacles without a cover plate installed.    c) Facility wide - On or about March 20, 2012, employees were working in and around future receptacles with energized 110 VAC wires exposed.
