29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: The employer does not protect employees from falling when working on slippery surfaces. This violation occurred on or about April 4, 2012, at the FMC Technologies where employees were exposed to a slip and fall hazard when performing mopping, stripping and rewaxing operations without a slip resistant footwear. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when exposed to slippery working conditions.

29 CFR 1910.132(h)(1): The employer did not provide the protective equipment, including personal protective equipment (PPE), used to comply with this part, at no cost to employees. The employer does not pay for specialty footwear, such as slip resistant footwear, to protect employees from falling when working on slippery surfaces. This violation occurred on or about April 4, 2012, at the FMC Technologies work site where employees were exposed to slip and fall hazards when performing mopping, stripping and rewaxing floor operations without slip resistant footwear.
