Jobsite: An employee did not wear a hard hat while working below two roofers who were removing shingles from the roof using shovels.

29 CFR 1926.451(e): When scaffold platforms more than 2 feet above or below an access point, portable ladder, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structural personnel hoist, or similar surface were not used. Crossbraces should not be used as a means of access. Jobsite: ladder jack scaffold on west side of building. A safe means to access the ladder jack scaffold was not provided. The work platform was postitioned on the 'climb' side of the ladders and employees had to climb over the platform to descend the ladders.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. Jobsite: An employee was exposed to a fall hazard of approximately 10.4 feet while working from a ladder jack scaffold work platform.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b) Note: If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502. Jobsite: Garage roof and house roof. Employees were not protected against falls while stripping shingles for the garage and house roofs. The employees were exposed to fall hazards in excess of 7.7 feet on the garage roof and 10.8 feet on the house roof.
