29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). a) On or about April 5, 2012, employees were not protected against accidental contact with energized power lines while erecting, dismantling, and working from frame scaffolding within 3 feet of the power lines. The power line had a total voltage of 460 volts. No abatement certification or documentation is required for this item.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: a) On or about April 5, 2012, employees working from frame scaffolding were exposed to fall hazards of greater than 25 feet. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
